Identify Possible Adverse Event During Chart Review Screening Criteria

Was the admission unplanned and the result of the health care received during previous the 12 months?

Patient admission

• have an unplanned transfer to the intensive care unit from a general ward?

• have an unplanned transfer to another acute care hospital?

• suffer trauma or injury?

• have an adverse drug reaction?

• have a documented error in drug therapy?

• develop a hospital-acquired infection?

• have excessive pain that was not properly managed?

• suffer a psychological or social injury?

• have an acute myocardial infarction, cerebrovascular accident, or pulmonary embolus?

• develop a neurologic deficit that was not present on admission?

• have a cardiac or respiratory arrest?

• die unexpectedly?

• have an injury or complication following an abortion or labor and delivery ?

• have a 5 minute Apgar score < 6, if newborn?

• have an unplanned return to the operating room?

• have unplanned open surgery after a closed or laparoscopic surgery?

• have the unplanned removal, injury or repair of an organ during surgery or invasive procedure?

• have any other undesirable outcome?

Is there evidence of pending litigation?

Did patient or family express dissatisfaction with the care?

Was there a complaint about the care from nursing or medical staff?
