Age of the patient at symptom onset

Has the patient developed multifocal lytic bone lesions over time?

Do X-rays of the lesions show osteolysis with sclerosis?

Does the patient experience pain and swelling associated with the bone lesions?

Has the patient experienced recurrent episodes, with periodic exacerbations followed by remissions?

• sacroiliitis?

• arthritis?

• psoriasis or a pustular rash?

• inflammatory bowel disease?

Are cultures of the bone lesions positive for bacteria or fungi?

Does a histologic examination of a bone biopsy shows acute or subacute inflammation?

Are acute phase reactants elevated during an exacerbation?
