Does the health care worker have an infection with an agent which causes serious disease and which is potentially transmissible to others?

Has the practitioner consulted with a specialist in Infection Control, Infectious Diseases or Occupational Health?

Does the practitioner scrupulously comply with all routine infection control measures?

Has the practitioner restricted her/his practice to only non-exposure prone procedures?

Does the practitioner comply with other recommendations deemed appropriate and reasonable?

Does the practitioner make suitable adjustments to protective measures during periods of enhanced infectivity?

Is informed consent from patients obtained if risk thresholds for transmission may be exceeded?

Is confidentiality of practitioner's medical information maintained by co-workers and the health care organization?
